JOB SUMMARY:

Transports residents to physician appointments. Provides transportation for residents upon discharge from hospital. Transports residents to and from scheduled outside activities.

GENERAL HEALTH REQUIREMENTS:

The necessary 2-step tuberculosis test upon hire.

GENERAL RESPONSIBILITIES:

• Reports any accidents or incidents to Care Coordinator or Administration.

• Applies Home’s accident, fire safety and emergency procedures for the protection of the residents.

• Responsible for careful treatment of Home’s equipment and utilization of supplies.

• Responsible for adhering to all Home policies.

• Knowing and protecting the resident’s privacy rights under the HIPPA regulations.

TRANSPORTATION EXPECTATIONS:

• Maintains an excellent driving record.

• Must report any accidents or traffic violations that occur on or off the job.

• If there is an accident involving company vehicle, whether major or minor in nature, the police must be called immediately and a police report filed. The Home is to be called as soon as possible.

• Maintains a valid driver’s license.

• Maintains private car insurance.

• Accompanies residents to physician’s appointments.

• Responsible for presentation and return of medical chart upon day of resident’s physician’s appointment.

• Maintains confidentiality of resident information, protecting and preserving such information from unauthorized use and disclosure.

• Helps maintain the self-respect, personal dignity, and physical safety of each resident.

• Clean out the vehicle(s) daily. Make sure they are free of trash and debris.

• Maintain an accurate Transportation Log that includes mileage and driver’s time for use of the company vehicles.

• Knowing and protecting the resident’s privacy rights under the HIPPA regulations.
